Insurance Information

Vision Insurance

The knowledgeable and experienced staff at Eyes For Life works directly with your insurance provider, verifying coverages and submitted claims regarding your vision care on approved benefits. We are more than happy to review your benefits and any out-of-pocket expenses with you before you receive treatment. Although many insurances cover the basic vision care costs, some treatments and/or products may not be covered by your insurance. There may be co-payments or other additional costs as stated by your insurance provider.

If you do not have insurance, or if insurance does not cover the services being provided, we offer a 15% discount off our professional fees, except for contact lens services and Optomap Imagery. This discount only applies if you pay same day, at the time of check out.

Please feel free to contact our office, and one of our Insurance Specialists would be delighted to go over your plan coverage and benefits with you.

Flexible Spending Accounts (FSAs)

Flexible Spending Accounts are a growing benefit option offered by many of today’s employers. FSAs are allow you to accrue money in a special medical savings account, pre-tax, to help you pay for medical expenses such as eye exams, glasses, contacts, and more. If you are unsure whether you have this option, ask the benefits administrator at your place of employment. FSAs can generally be used in conjunction with vision coverage to offset any out-of-pocket expenses.

Eyes For Life accepts many insurance carriers, some of which are featured below. If you don’t see your insurance listed, please contact us to see if we work with your provider or vision care plan. We will also verify your insurance benefits for you!
